Why is cookie called biscuit?

Biscuit means twice baked. You generally bake the dough. Then, to make it crispy, you bake it again for a few minutes. Biscotti is the thing most of us are familiar with that still requires this procedure. But most small baked items...cookies, muffins etc. are not twice baked. In England, the word is still used to mean any small, sweet, and hard-baked item.
